If you are paying for a partner, enter their name too.
We only allow for Quantity of 1, so you will need to calculate and enter the total amount that you are paying. Enter this amount into the price per item box.

In the purchase details description box, Type: "Event Name and Your Name". Thank You!

2024 SCLHTG Membership:

SCLHTG Membership Mail-in Form, print, fill in the blanks and mail it to LHTG, PO Box # 1535, Lincoln CA 95648 with your $20 check.
